NEW PATIENT REGISTRATION
The practice accepts patients living in the practice area. Proof of address is required.
The practice does not discriminate on grounds of race, gender, social class, religion, sexual orientation, appearance, disability or medical condition.
Patients are registered with 'Inverkeithing Medical Group' and have the choice to see any of the GP's in the Practice.
The registration process requires completion of forms GPR and NP1 and Online Patient Access/Text Messaging (if you want this service).
If you do not have access to the internet, these forms are available from reception.
Please complete these forms fully and return to fife.img@nhs.scot or hand into reception.
